How much does a hair transplant cost in Portugal?

At Replace Clinic we work with fixed pricing by follicle tier, set according to the average quantity needed for each degree of intervention. The price doesn't depend on haggling or per-follicle calculations: it's presented in writing at the assessment consultation, already including the surgical plan, the proposed technique and the 18 months of follow-up. This policy exists to remove surprises and restore trust in a market where opaque quotes and promotional prices are often used to attract patients.

Clinically reviewed by Dr Fátima Garcês, Clinical Director of Replace Clinic — 20 years dedicated to hair restoration.

Key points

  • Fixed price by follicle tier: the amount is set before the procedure

  • Written quote at the consultation, with 18 months of follow-up included

  • No hidden costs: what's quoted is what's charged

How our fixed pricing by tier works

The cost of a hair transplant at Replace Clinic is calculated by follicle tiers, according to the area to be covered and the expected density. Rather than charging per individual follicle — which makes budgeting unpredictable — we group procedures into tiers based on the average number of follicles required.

During the assessment consultation, Dr Fátima Garcês determines which tier your case falls into and presents the corresponding amount. If, for clinical reasons, more follicles are needed than expected within the same tier, the price doesn't change.

  • Initial tier: more localised procedures, such as correcting the temples or small areas of thinning.
  • Intermediate tier: moderate scalp coverage, usually associated with 3,000 to 4,000 follicles.
  • Advanced tier: extensive cases, requiring 4,000 to 6,000 follicles or more, sometimes spread across two sessions.
  • Each tier's price is fixed and includes surgery, basic medication, follow-up consultations, and 18 months of aftercare.

Why we choose fixed pricing

The hair transplant market is often associated with less-than-transparent practices: per-follicle prices that rise on the day of surgery, hair-tourism packages with hidden costs, and proposals that don't include post-operative follow-up. Replace Clinic adopts fixed pricing specifically to reverse that pattern.

A fixed price by tier lets the patient know, before booking surgery, the maximum investment their case will require. There are no surcharges for extra follicles within the same tier, and no additional charges for follow-up consultations or complementary treatments during the first year and a half.

What's included in the quote

All Replace Clinic quotes are presented in writing and include the same elements, regardless of the tier. The difference between proposals from other clinics isn't always in the final figure — it's in what that figure actually represents.

  • Assessment consultation with trichoscopy and definition of the surgical plan.
  • Procedure carried out by Dr Fátima Garcês, with a dedicated team.
  • FUE technique with no scalpel, no stitches and no linear scar.
  • Basic medication and post-operative care kit.
  • Clinical follow-up and photographic record over 18 months.
  • No hidden costs: the amount presented at the consultation is the final amount.

How to compare quotes from different clinics

The difference between two quotes is rarely just the number. It's in what each clinic includes, and in who actually carries out the procedure.

  • Check whether the price is fixed by tier or varies with the exact number of follicles implanted.
  • Ask who performs the extraction and implantation: the doctor or only technicians.
  • Check whether follow-up consultations, medication and complementary treatments are included.
  • Ask for photographs of real cases from the clinic itself, with a similar degree of hair loss to your own.
  • Be wary of hair-tourism packages priced well below the market: correcting a poor result usually costs more than the original procedure.

Alternatives when a transplant isn't the right step

Not every case initially benefits from surgery. In cases of active shedding, the first step is to stabilise the follicle with medical treatment — hair mesotherapy, PRP, photomedicine and a nutritional protocol — which represents a significantly lower investment.

When the donor area is insufficient for FUE, the Biofibre® Hair Implant, for which we are the exclusive representative in Portugal, can restore density with a different cost structure, based on the number of fibres implanted.

Frequently asked questions

What's the average price of a hair transplant in Portugal?
At Replace Clinic the price is fixed by follicle tier and is presented in writing at the assessment consultation. We don't charge per individual follicle, so there are no surprises on the day of surgery. The quote also includes 18 months of clinical follow-up.
Can the price go up if more follicles are needed?
Within the tier set at the assessment, the price doesn't change. If the number of follicles required exceeds the initial tier's limit, the case is moved to the next tier, also with a fixed price communicated before the procedure.
Is a hair transplant covered by the NHS or health insurance in Portugal?
Generally not, as it's considered a cosmetic procedure. There are exceptions in reconstructive cases, such as burn or trauma scars — you should check the conditions with your insurer.
Can I pay in instalments?
Yes, staged payment options are available. The available terms are presented along with the quote, at the end of the assessment consultation.
Does the assessment consultation have a cost?
The first assessment can be booked in person in Lisbon or by videoconsultation. It includes trichoscopy, case analysis and delivery of the plan with the quote.
Is it worth having a hair transplant abroad because it's cheaper?
A lower price usually reflects less time per patient, teams without direct medical supervision, and no post-operative follow-up. Corrective procedures to fix poor density or a badly designed hairline often end up costing more overall.
